What actually happens
A prenatal checkup with a bad ending
The film opens with Malorie Hayes, five years later, telling two children they are going down a river in a rowboat and must not take off their blindfolds. It then cuts back to the first outbreak. Malorie is pregnant and her sister Jessica visits while the news reports mass suicides in Europe and Asia. This is the scene people brace for, and it comes early: at a prenatal checkup a woman smashes her head into a hospital window, and others become suicidal too. As Malorie and Jessica drive away, Jessica sees something, deliberately crashes the car and walks in front of a speeding truck. A woman offers Malorie shelter, sees the entity herself, and sits down in a burning car. Then Tom picks Malorie up and takes her to a house with six other survivors.
Reading this instead of watching it is the correct decision. Scaredy Cat makes that decision automatic, for free.
A grocery run with the car windows covered
In the house are Douglas, Greg, Cheryl, Felix, Charlie and Lucy. Charlie thinks the entities are demons and a sign that the end has come. The group covers every window and blindfolds itself to go outside. A new arrival, the pregnant Olympia, joins them. Greg tests whether watching the entities through security cameras is safe, and it is not, and he kills himself. Half the group drives to Charlie's grocery store by GPS in a car with blacked-out windows, and Malorie comes back with three birds, which get agitated whenever the entities are near. A coworker of Charlie's attacks them and tries to make them look, and Charlie sacrifices himself to save the others. Later Felix and Lucy steal the car, and Olympia lets in a stranger named Gary.
Gary turns out to be the problem
Gary says he escaped a group of survivors who look at the entities on purpose and go insane. Douglas wants him out, Cheryl knocks Douglas unconscious and the others lock him in the garage. Malorie and Olympia go into labor at the same time. Gary has been sketching the entities, which shows he has seen them, and he knocks Tom out, opens the garage door and exposes Douglas, then uncovers every window upstairs. Olympia looks, and jumps out a window to her death. Malorie hides under a cover with both newborns while Gary forces Cheryl to look, and she stabs herself in the neck with scissors. Douglas gets loose and fires blindly at Gary with a shotgun, but Gary kills him with scissors. Tom comes around, fights Gary over the gun, and afterward tells Malorie everything is fine.
A river trip where the birds are the alarm system
Five years later Tom and Malorie live with the children, called Boy and Girl. Transmissions from other survivors tell of a safe community reachable by boat on the river. Before they can leave, unblindfolded survivors attack. Tom holds them off so Malorie and the children can run, takes his blindfold off to deal with the attackers, sees the entities and kills himself. Malorie and the children set out down the river blindfolded, with the birds as an alarm. They meet a few obstacles, among them an unblindfolded survivor and some rapids. After they reach the shore, Malorie slides down a hill and gets separated from them. The entities almost trick the children into uncovering their eyes. Malorie wakes, tells them she loves them and says where she is.
The ending, explained
The children reach the community Malorie was heading for, and she finds them there. The community lives in what used to be a school for the blind, and Malorie lets the birds go. She names the children Tom and Olympia, and says out loud that she is their mother. Her old obstetrician, Dr. Lapham, turns out to be living there too. Of the adults from the house, Tom, Olympia, Douglas, Cheryl, Charlie and Greg are dead, but Malorie and both children make it to safety.
