What actually happens
An audition and an ex-prima ballerina's bad week
Nina Sayers dances for the New York City Ballet and lives with her mother, Erica, a former ballerina who watches her closely. The company is opening its season with Swan Lake. Beth, the prima ballerina, is pushed into retirement, so the director, Thomas Leroy, needs a dancer for the White Swan, Odette, and the Black Swan, Odile. Nina nails Odette and cannot manage Odile, and Thomas dismisses her. She asks him to reconsider the next day, he forces a kiss on her, and she bites him and runs. Surprisingly she gets the role anyway. At the gala, a drunk Beth accuses Nina of trading favors for the part. Beth is hit by a car the next day, and Nina visits her in the hospital and sees that her legs are too damaged for her to ever dance again.

A late night with Lily
Thomas tells Nina to watch Lily, a newcomer who looks like her but is much looser on stage. Nina begins to hallucinate and finds scratches on her back. Against her mother's wishes, she goes out drinking with Lily, who slips ecstasy into her drink after Nina turns it down. A wild night follows and ends in Nina's bedroom with Lily. Nina wakes up alone, late for rehearsal, and Lily later denies that anything happened. When Thomas names Lily as Nina's alternate, Nina decides Lily is out to take her place. Her injuries and visions get worse, and at one point she believes she is turning into Odile. Erica worries and tries to stop Nina from dancing on opening night.
Opening night, with a mirror
After a scuffle with Erica and a run-in with Beth, Nina reaches the theater and finds Lily about to dance Odette in her place. She talks Thomas into giving the part back. Near the end of the second act a hallucination throws her off, and her partner drops her during a lift, which infuriates Thomas. Back in the dressing room, the scene people dread most arrives, late in the movie: Lily turns into a copy of Nina, the two fight, and a mirror breaks. Nina stabs the double with a piece of glass, and the illusion ends. Nina then hides the body. She dances Odile flawlessly while feathers appear on her arms. The audience stands, and Nina kisses Thomas.
The final act, in a white costume
Back in her dressing room, Nina changes into the Odette costume and white makeup. Lily comes in to congratulate her, alive. The mirror is still broken, but the body and every trace of the stabbing are gone. Nina glances down, finds a shard of glass in her own stomach, and understands that she stabbed herself. She dances the last act anyway, with blood slowly soaking through her costume. Odette ends the ballet by throwing herself off a cliff, so Nina jumps onto a hidden mattress. The theater erupts in applause. Thomas, Lily and the other dancers gather backstage to congratulate her, and Thomas finally sees that Nina is bleeding and calls for help. Nina says that she felt it and that she was perfect, and the screen fades to white.
The ending, explained
Nina did not stab Lily. She stabbed herself with a shard of the broken mirror and then danced the rest of the ballet, with blood seeping through her white costume. Lily is alive and congratulates her. The show ends with Odette jumping off a cliff, and Nina does the jump onto a hidden mattress. Thomas notices the blood backstage and shouts for help. Nina says she felt it, that it was perfect, and that she was perfect. The screen goes white without showing whether she survives. Beth lives but will never dance again.
