What actually happens
Becca inherits a castle in Albania
Becca Riley lost her sight when her boyfriend John caused an accident by driving under the influence, and the strain shows in their relationship. Then an estate agent named Marku tells Becca that her birth mother, Lavinia Whateley, has died and left her a castle in Albania. John wants to sell it fast and go home, while Becca wants to find out who her mother was. Neither of them knows that Lavinia kept a monstrous creature chained in the castle, and that someone has recently let it loose. Once they arrive, Becca hears odd noises and has visions, a few of them sent by the creature. John won't believe any of it, and that makes things worse between them.

The journal and the Necronomicon
John has invited four other people to the castle, including Shelly, whom he has been flirting with, and the Professor, who studied the occult at Miskatonic University and is the only one who takes Becca's sounds seriously. While exploring, they find a Necronomicon and a network of tunnels that leads to Lavinia's journal. It tells how Lavinia's own father forced her into pregnancy during a ritual to Yog-Sothoth, and she had twin girls. Becca was adopted out, and her sister was born deformed and held captive. Lavinia warns that if the two ever meet and clasp hands, a gate to Yog-Sothoth will open.
The creature in the tunnels
Becca and her sister get separated in the tunnels, and the Professor meets the sister first. He talks her into giving him lessons on the Elder Gods, and the lessons convert him into a cultist. Meanwhile the creature hunts the other guests one at a time, and it rapes John. That is the scene people dread most, and it happens in the middle of the movie while the guests are split up in the tunnels. John ends up as the last guest alive, wandering the tunnels. He finds Marku held captive and lets him go, and Marku admits it was all arranged to bring the sisters together, then attacks John. John kills him in self-defense and runs, but the creature catches up. John fights back and is about to kill it when Becca arrives and begs him to stop. He hits Becca, she injures him, and the creature finishes him off.
The ending, explained
Becca is overwhelmed, and her sister leads her to the ritual site while the Professor urges her to accept her destiny. Becca takes her sister's hand, which opens the gate and lets Yog-Sothoth into the world, and Becca's body begins a monstrous transformation. John and Marku die, and the creature picks off the other guests. After the credits, the Professor walks into his office at the university, where someone calls him Mr. Armitage. He greets a man he calls West, and a glowing vial sits on the desk.
The post-credits ruling
There is a scene after the credits. The Professor walks into his university office, is called Mr. Armitage, and greets a man he calls West while a glowing vial sits on the desk.
