An anonymous gift arrives for Jamie Ashen and his wife Lisa: a ventriloquist doll called Billy, marked as number 57. Jamie goes out, a figure kills Lisa, and he comes home to find her dead with her tongue cut out. Detective Jim Lipton arrests and questions him, then lets him go for lack of evidence. In Billy's box Jamie finds a message about Mary Shaw, a dead ventriloquist from Raven's Fair, the town where he grew up.

A hometown that has not aged well
Raven's Fair is old and run-down. Jamie calls on his disabled father Edward, whom he is estranged from, and Edward's much younger wife Ella, and asks the local mortician Henry Walker to help with Lisa's funeral. Henry's senile wife Marion tells Jamie to bury Billy, so he does, and Lipton turns up and finds this suspicious. Henry then tells Jamie about Mary Shaw. A young boy named Michael said he could see her lips moving during a performance, and she was humiliated. When Michael went missing, his family blamed Mary and lynched her. Mary wanted to be made into a doll and laid to rest among her own dolls. Henry saw her rise as a child and was spared because he stayed silent, since Mary only kills people who scream. Jamie learns that Michael was his great-uncle, that Mary had really murdered him, and that the Ashen family silenced her by cutting out her tongue. She has been killing them the same way ever since.
A theater with a full display case
Mary kills Henry, and Lipton finds out that every one of her dolls has been dug up. In Mary's old theater, Jamie and Lipton find a hundred dolls in their display case, along with Michael's body, which has been turned into a marionette. The case meant for Billy is empty, which shows that Jamie did not put Billy in his own home. Mary's ghost tells Jamie she killed Lisa because Lisa was pregnant, which ended any child the Ashen family might have had. Jamie and Lipton set fire to the dolls to destroy them, and Lipton trips and screams, which seals his fate.
Billy goes in the fireplace
Back at his father's house, Jamie faces Mary and drives her off by throwing Billy into the fireplace. Then he learns his father is already dead. The Edward he has been talking to is a doll made from his corpse, and Ella has been supplying the voice, because she is the perfect doll Mary made before she died. This is the scene people dread most, near the end, so here is exactly what happens: Jamie screams as Mary possesses Ella, and he meets the same fate as the others.
The ending, explained
Jamie screams when Mary possesses Ella, and Mary kills him the way she killed the rest of the Ashen family. Afterward Mary pages through an album of photos of her victims: Jamie, Lisa, Lipton, Henry and Edward, with Ella last. Then she shuts the album, her revenge complete, and that is where the film ends.
