A box addressed to the boss
Paxton, who escaped the Elite Hunting Club, lives in seclusion with his girlfriend Stephanie and suffers from PTSD. After a fight in which she calls his paranoia exaggerated, Stephanie wakes up to find his headless body in the kitchen. Soon after, an unmarked box holding his head arrives for Sasha Rassimov, who runs the club. Elsewhere, in Rome, three American art students, Beth, Whitney and Lorna, are talked into a spa vacation in Slovakia by Axelle, a model they have been sketching. On the train, Beth is bothered by a stranger, and Lorna's iPod is stolen and recovered by Axelle. At the hostel, the desk clerk quietly posts their passport photos to the club's auction site. Todd, an American businessman, and Stuart, his closest friend, place the winning bids for Whitney and Beth and head for Slovakia.

A harvest festival with an unlisted boat ride
At the village harvest festival that night, a local man named Roman invites Lorna on a boat ride downstream. She is kidnapped and wakes up bound and hanging upside down. A woman named Mrs. Bathory comes in, undresses, lies underneath her and cuts her with a scythe, ending with her throat. Meanwhile Beth, Whitney, Axelle and a local named Miroslav go to a spa. Beth falls asleep and wakes up alone, without her things, with several men chasing her. In the woods a gang of local children ambushes her, until Sasha and Axelle arrive, and once the two women are gone Sasha punishes one of the boys by killing him. At Sasha's mansion Beth realizes the men chasing her work for him and Axelle. She finds a room of trophy heads, one of them Paxton's, and is captured and tied up in a room at the Art Show. Stuart comes in and seems reluctant to kill her, but he knocks her out after untying her.
A contract with no refund policy
Whitney bites off the makeup artist's nose and tries to run, but she is caught. In another room Todd threatens her with a power saw, hurts her by accident and loses his nerve. He tries to leave, and the club tells him the contract requires him to finish. Todd refuses, so the guards set attack dogs on him, and they tear him apart. The club then offers the badly injured Whitney to its other clients, including an elderly Italian man who is busy eating Miroslav alive. Stuart does not know how Todd died. He shows Beth photos of Whitney to frighten her, accepts the club's offer and beheads Whitney.
Beth reads the fine print
Stuart prepares to torture Beth, speaking to her as though she were his wife. She seduces him into untying her, then he orders her to the floor and tries to rape her. She fights him off and fastens him to the chair instead. Sasha and the guards arrive, and Beth tries to pay for her release out of her inheritance. Stuart tries to top her offer, but Sasha knows Stuart cannot pay, since Todd covered the cost of both. Sasha says Beth has to kill someone before she can leave. Stuart insults her, and this is the scene people dread most, near the end. Beth cuts off his genitals and leaves him to bleed to death. Even the jaded guards stop for a moment. Satisfied, Sasha gives Beth the club's tattoo.
The ending, explained
That night, the local children lure Axelle into the woods, where Beth is waiting for her. Beth beheads her, and the children start playing football with the head. Beth is the only one of the three students who gets out. Lorna and Whitney are dead, Todd and Stuart died in the club's rooms, Miroslav was eaten alive, and Paxton was dead before the students ever left Rome.
