What actually happens
A health inspection with a rosary
A man called Dr. Owen, who represents the Ministry of Health, takes a heavily armed four-person GEO team (Fernandez, Martos, Rosso and Larra) into an apartment building under quarantine. A virus has turned the residents violent, and the team carries video cameras and body cams. The attic is empty, and a disturbance on a lower floor ends with infected residents attacking the group. Martos is infected, and Owen holds him off with a rosary and a religious mantra, which is the first sign that Owen is not only a health official. He admits he is a priest sent by the Vatican to collect a blood sample from Tristana Medeiros, a possessed girl who is where the infection began. The team finds the body of Father Albelda, the priest who handled her case first, and a sample of her blood that he took, which they then lose. Owen says they will need blood from Tristana herself. Another attack follows, and Larra, cornered, takes his own life.

The sewer is a poor shortcut
Outside, the father of an infected girl named Jennifer talks a firefighter into taking him into the building through the sewers. Three teenagers named Tito, Mire and Ori follow them without being noticed, and the police seal the exit behind everyone. Jennifer's father is infected and the GEO team kills him. Mire accidentally kills the firefighter, and then kills Martos as well. When Tito becomes infected, the team locks Mire and Ori in a room. Then they run into the reporter Ángela Vidal, who is shaken but unhurt. The group questions the possessed Tito, and from his cryptic answers they work out that Medeiros is in the penthouse and that some parts of the building can only be reached in total darkness with infrared light.
A door that only opens for night vision
Using the night vision on Ángela's camera, the group finds a hidden door. This is the scene people brace for, and it comes near the end. Medeiros attacks, kills Fernandez and goes for Owen, and Ángela shoots her with a shotgun. That is not the end of it. Owen is furious, because he needed the blood sample, and he refuses to let anyone leave. Ángela attacks him, and when Rosso tries to step in, she kills him. The demon that possessed Tristana has moved into Ángela. The possession happens right after the shotgun blast, with no warning in between.
The ending, explained
Ángela kills Owen and then phones the authorities using his voice. She tells them the mission is accomplished, that Owen has to stay inside because he is infected, and that the only survivor allowed out is Ángela. A flashback then shows what happened just before the movie started. Medeiros had captured Ángela and forced a worm-like organism into her through the mouth, and when the response team arrived, Ángela went into hiding. By the end, Medeiros, Owen and the GEO team are dead, and the demon is the one making the phone call.
