What actually happens
A body with unfinished business
After Sienna beheads him at the end of the last movie, Art the Clown's body gets up again inside the haunted house. It strangles and decapitates a police officer and heads to the asylum, where the survivor Victoria Heyes, who is possessed by the Little Pale Girl, has delivered Art's head. Art puts the head back on, and together they dispatch a nurse and a guard before settling into an abandoned house to hibernate. There, Victoria is forced to take her own life so that the Little Pale Girl can completely take over her body. Half a decade on, Sienna leaves a mental health center to move in with relatives: her aunt Jess, uncle Greg and cousin Gabbie, who admires her but knows nothing about what she went through. Sienna carries survivor's guilt and hallucinates the people Art killed, including her friend Brooke. She is also struggling to reconnect with her brother Jonathan, who is now in college.
Reading this instead of watching it is the correct decision. Scaredy Cat makes that decision automatic, for free.
The Santa suit changes hands
Two demolition workers wake Art and Victoria, and both workers are killed. Art then shoots two men, freezes a Santa Claus impersonator at a bar with liquid nitrogen and takes his costume. At 3 a.m. he breaks into the house of an innocent family and kills them with an axe. The next morning he works as a mall Santa and hands out explosives disguised as presents, and several people die, many of them children. Sienna believes she sees Art in costume at the mall just before the blast, but she doesn't tell her family, because she's afraid they won't believe her.
A podcast interview nobody agreed to
Sienna visits Jonathan at his university and meets his roommate Cole and his girlfriend Mia, who runs a podcast about true crime. Mia pushes Sienna and Jonathan to do an interview about the Miles County Massacre, and Sienna snaps at her. Sienna tells Jonathan she thinks Art is alive and shows him a letter he wrote her in the psychiatric hospital, which says demons can enter this world by possessing recently dead evil people, such as serial killers. She plans to go back to the Terrifier carnival to get their father's sword. Art shows up on campus and uses a chainsaw on Mia and Cole while they are in the shower. When Sienna sees the mall killings on the news, she tells Jonathan to come home and warns her family that they aren't safe. Greg goes to pick Jonathan up, and Sienna falls asleep and dreams of her father and of angels building the armor from the warrior costume he drew for her.
Rats in the dining room
This is the stretch people dread, so here it is in order. Sienna wakes to Jess and Greg downstairs, discussing what to do about her worsening condition. In the dining room she finds Art and Victoria pinning Greg's decapitated body to the wall, and Art knocks her out with a mallet. She comes to tied to a chair next to Jess, both with their mouths taped. Victoria shows them a skinless head being eaten by rats and says it is Gabbie's. Art and Victoria then kill Jess: they hammer a tube into her throat, push rats down it and slit her throat. Art brings in Gabbie alive as a hostage, and Victoria changes her story and says the head belongs to Jonathan.
The wrapped present
Victoria puts a crown of thorns on Sienna and tries to possess her, but Sienna resists. Before Sienna and Gabbie can be sentenced to death, Gabbie begs Sienna to open a present she brought, and Victoria has no idea it holds Sienna's sword. Art crushes both of her hands with a blow, then cuts her loose so she can open it. Sienna swings the sword, slicing Art's throat and taking off Victoria's head. While Art and Sienna fight, Victoria's body dissolves into demonic blood that eats through the floor and opens a gateway to Hell. Gabbie is in danger, and despite Sienna's efforts she falls into the abyss, taking the sword with her.
The ending, explained
Art sees his chance and escapes through a window. Sienna looks down at her wounded hands, which are healing, and vows to find Gabbie and bring her back. Greg and Jess are dead, and Jonathan's fate is left open beyond Victoria's claim about his head. Later, a bloodied Art boards a city bus, sitting near a woman who is reading a book called The 9th Circle. Victoria is dead and her remains opened the portal that took Gabbie, so Sienna's next move is to go after her.
