What actually happens
A search party led by the prime suspect
Two days after the first film, Sarah staggers out of the cave system covered in blood, with amnesia. At the hospital a doctor finds that some of the blood matches the type of her missing friend Juno. Sheriff Vaines and his deputy Rios take Sarah back toward the cave along with three pothole specialists named Dan, Greg and Cath, and they reach it through an old mine shaft run by an old man named Ed. Near the entrance they find Rebecca's corpse, which sets off flashbacks for Sarah. Vaines suspects she is responsible for the women's disappearance. Partway through a tunnel, Sarah lashes out at Vaines and Greg and then bolts.

Never raise your voice underground
The team splits up to look for Sarah. A crawler startles Vaines, his pistol goes off, and part of the cavern collapses and pins Cath under rocks away from the others. The rest look for another route and enter a room full of bones, where they find Holly's damaged video camera. The recordings show that the missing women were attacked by crawlers. Sarah is hiding close by, overhears the footage and recovers her memory. Rios panics and calls for help, which brings the crawlers toward her. Sarah clamps a hand over Rios's mouth, since the crawlers are blind and hunt by sound, and they watch a crawler kill Dan and drag him away. Cath squeezes free, crushes a crawler under the rocks and meets Greg. They find Sam's body hanging across a chasm and try to swing over on it, but crawlers attack, Greg sacrifices himself, and Cath is killed after she screams his name.
The handcuff was a mistake
Vaines is attacked by a crawler, and Juno, who is still alive, rescues him. In a pool of water, Sarah and Rios take down a crawler, and then they meet up with Vaines and Juno. Juno is angry that Sarah stabbed her leg with a pickaxe and abandoned her, and she nearly kills Sarah until Rios lies that Sarah came back voluntarily. Juno leads them to a feeding pit with a supposed passage to the surface. Vaines handcuffs himself to Sarah so she cannot run again, and then he falls over a ledge and nearly pulls her down with him. With crawlers closing in, Juno tells Rios to remove Vaines's hand. Rios does it despite his protests, which lets Sarah go while Vaines and the crawlers on him fall to their deaths. That is the worst moment, and it happens in the middle of the movie.
Ed and his shovel
Sarah, Juno and Rios reach the exit and find a group of crawlers and their huge leader in the way. They try to sneak past, but the dying Greg turns up and grabs Juno's leg to save himself. She screams, and the crawlers come. Greg dies, and the women fight again. Once the other crawlers are dead, Sarah tries to pull Juno away from the leader, which slashes Juno's stomach and wounds her fatally. Sarah kills it, and Juno dies in her arms. More crawlers arrive, and Sarah screams to draw them toward herself so Rios can escape. Rios climbs out and tries to call for help, but Ed knocks her out with a shovel and drags her back to the cave entrance. As she wakes up, a bloody crawler comes out of the cave with its arms stretched wide.
The ending, explained
Rebecca, Dan, Cath, Greg, Sam, Vaines and Juno are all dead by the end. Sarah screams to pull the crawlers away from Rios, which lets Rios get out of the cave. Outside, Ed knocks Rios out with a shovel and leaves her at the cave entrance. She is just coming around when a blood-covered crawler walks out of the cave with its arms outstretched.
