What actually happens
A floor plan that faces the wrong way
Beth's husband Owen has died by suicide, and she spends her nights drinking and sorting through his belongings. Her friend Claire and her neighbor Mel both worry about her. Owen left a note telling her she had been right, that nothing is after her and that she is safe now, and she cannot make sense of it. Strange things start happening at night, and she finds a floor plan of their house drawn in reverse. Then she finds a picture on his phone of a woman with a strong resemblance to her, and decides he was having an affair. Drunk, she tells Claire that a car accident years ago left her dead for four minutes, and that all she saw was nothing. Owen disagreed and had nearly talked her out of it. That night she watches frightened women run through the woods into the lake, spots blood in the boat where Owen ended his life with a gun, and feels something invisible in the room.
Beth rows across the lake toward some odd lights and finds a mirror-image copy of her own house, with ghostly women standing around Owen. She passes out and wakes up at home. When she finds the reversed house again it is unfinished and empty, and she takes a strange statue out of it. Mel says he never saw any such house, but he did once see Owen out in the dark woods with a woman who resembled Beth. On Owen's laptop she finds photos of more lookalikes. She matches the statue to a book and learns it is an occult doll, and she concludes that Owen was studying ways to fool and capture demonic entities. She traces the books to a bookstore, where she meets Madelyne, one of the women in his photos. Madelyne denies sleeping with him and says Owen invited her to the reversed house, kissed her, and then tried to choke her before he apologized and drove her home.
Floorboards in the wrong house
A drunk Beth goes back to the reversed house, and this time she looks under the floorboards. There she finds the bodies of the women Owen photographed, in the middle of the movie. After that the invisible presence starts touching her, and she hugs it back because she thinks it is Owen. It is not Owen. It shows her violent visions of Owen killing the lookalikes, and it is Death in person, and it goes by Nothing. Death had pushed Owen to send Beth back to the afterlife by killing her. Owen built the reversed house and murdered the lookalikes to fool it, and that worked for a while. Beth escaped it once in her near-death experience and it drove Owen to suicide, so now it is coming for her directly. It traps her in a pose like the statue's.
The ending, explained
Claire arrives the next morning and finds signs of a fight in the house. She runs to the dock with Mel, and they see Beth out on the boat with Owen's gun. In Nothing's own dimension, the entity urges Beth to join it by shooting herself, but she lowers the gun. The moment she does, she is back in the real world and Claire is swimming out to rescue her. On shore, Beth can see the entity's outline in the boat. Mel says there is nothing there, and Beth answers that she knows.
