What actually happens
A lakeside house with a very leaky bathroom
Claire Spencer used to be a cellist, and she lives in a lakeside house in Vermont with her husband Norman, a scientist and professor. Things between them are tense. Claire's daughter Caitlin, who is Norman's stepdaughter, has just left for college, and a year has passed since Claire's serious car accident. Claire watches their new neighbors, Mary and Warren Feur, argue, and when Mary vanishes for several days, Claire suspects Warren has killed her. Soon strange things start happening at home. Claire believes she spots a woman's body in the lake, the front door opens by itself, and she feels something watching her. A framed newspaper article about Norman falls off his desk and breaks, and Claire finds an odd key. Then she walks into the bathroom to find the bathtub full of water and a woman's reflection looking back at her.
Reading this instead of watching it is the correct decision. Scaredy Cat makes that decision automatic, for free.
A séance that solves the wrong problem
Norman does not believe the house is haunted and tells Claire to see a therapist about her anxiety. In therapy, Claire mentions that the woman she sees looks like her. She and her friend Jody also hold a séance. Afterward the bathtub is full again, a message on the fogged-up bathroom mirror says "You're next" and then "you know," and her computer types the letters MEF. Claire decides Mary's ghost is behind it and goes to confront Warren, and Mary steps out beside him, alive and well. At a party, a friend brings up a fight Claire and Norman had a year ago and asks if she is all right. At home, the clipping falls again, and the reverse side carries a report on Madison Elizabeth Frank, a woman who disappeared and who looks a lot like Claire. Claire calls on Madison's mother and quietly pockets a lock of Madison's hair. She then tries to summon Madison's ghost and is possessed by it, seducing Norman and speaking in Madison's voice. It stops once Claire lets go of the hair, and then she remembers that Madison was one of Norman's students and that the two of them had an affair, a fact the accident had wiped from her memory.
A town called Adamant, which Norman pretends not to know
Claire spends the night at Jody's house, where Jody says she once watched Norman argue with a woman at a café in Adamant, a town nearby. When Claire gets home, Norman is lying unconscious in the bathtub and a hairdryer is plugged in nearby. He says it was an accident. He admits Madison confronted him when he ended the affair, but says he did not kill her. Later, at the lake, an unseen force pulls Claire under, and she sees a jewelry box that matches Madison's necklace. Norman pulls her out, and they burn the lock of hair. They seem to patch things up, but when Claire brings up visiting Adamant, Norman acts as though he has never heard of it. She goes alone, recognizes the necklace and the jewelry box in a shop window, and recovers the box from the lake. The key from Norman's office opens it, and Madison's necklace is inside.
The bathtub scene, and the lake scene after it
This is the part to brace for, and it comes near the end. Norman says Madison killed herself and that he pushed her car into the lake, and Claire insists that he call the police. Then she finds out the call was fake. Norman paralyzes her with halothane and admits he murdered Madison to keep her from telling the dean about the affair. He puts Claire in the bathtub and runs the water to stage a suicide. When he takes off the necklace, Claire's face briefly turns into Madison's corpse, he stumbles backward, and he hits his head on the sink. As the drug wears off, Claire pulls the plug and gets away. Norman wakes up and chases her, and they crash his truck into the lake. Madison's car sits below them, and the wreck shifts, dislodging her corpse, which grabs Norman. Claire escapes while he drowns.
The ending, explained
The truck crashes into the lake with Norman trying to drown Claire inside it. The wreck of Madison's car shifts and a piece of the boat breaks through the roof, which frees Madison's decayed corpse. It floats up, appears to come alive, and grabs Norman, who drowns while Claire gets away. Madison's body goes still again and sinks, and her necklace slips out of Norman's hand. When winter comes, Claire goes to the cemetery and leaves a red rose on Madison's grave. Claire survives and Norman does not.
