What actually happens
A gas station that doubles as a horror museum
On October 30, 1977, two amateur criminals, Karl and Richard Wick, try to rob a gas station that is also a horror museum, and Captain Spaulding, who owns the place, kills them along with his assistant Ravelli. Later that same night, four travelers, Jerry Goldsmith, Bill Hudley, Mary Knowles and Denise Willis are driving around looking for odd roadside attractions to put in a book. They meet Spaulding at his Museum of Monsters & Madmen and hear the local legend of Dr. Satan. They set off to find the tree where he was hanged and pick up a hitchhiker named Baby, who says she lives nearby. A hidden figure shoots out a tire with a shotgun. Baby leads Bill to her relatives' home to look for a tow truck, and her half-brother Rufus collects the other three and drives them there too.

A Halloween show with an audience that cannot leave
At the house they meet Baby's adopted brother Otis Driftwood, her half-brother Tiny, Mother Firefly and Grandpa Hugo. Over dinner, Mother Firefly tells them that her ex-husband Earl had a psychotic breakdown and tried to set Tiny and the house on fire. The family puts on a Halloween show, and Baby upsets Mary by flirting with Bill. Rufus says the car is fixed, but as the travelers leave, Otis and Tiny, dressed as scarecrows, jump them in the driveway and hold them prisoner. The following day Otis kills Bill and makes a Fiji Mermaid out of his body. Mary is bound in a room, Denise is dressed as a doll, and Jerry is partly scalped because he cannot name Baby's favorite movie star. Denise never gets home, so her father Don reports her missing. Deputies George Wydell and Steve Naish find the couples' car with a dead cheerleader in the trunk, and Don joins them. At the Firefly house, Mother Firefly shoots Wydell, and Otis kills Don and Steve.
Rabbit costumes for a trip to the well
That night the three remaining travelers are dressed as rabbits and taken to an abandoned well. Otis taunts Denise by wearing her dead father's facial skin as a mask. Mary tries to run, but Baby catches her and stabs her to death. The family piles the bodies onto a pyre and sets it alight. Jerry and Denise, who are left, are put in a coffin and lowered into the well, and that sends the story underground.
Dr. Satan receives visitors
Down in the well, Dr. Satan's failed experiments break the coffin open and drag Jerry away, leaving Denise to find her own way through an underground lair. The tunnels are full of mutilated corpses. Denise meets Dr. Satan, along with several mental patients. The scene people dread comes near the end: Jerry lies on Dr. Satan's operating table and is vivisected, and he dies as Denise screams. Then Dr. Satan sends his huge assistant after Denise, and the assistant is Earl, Mother Firefly's ex-husband. She tricks him and crawls to the surface, and Earl is crushed when the tunnel collapses.
The ending, explained
Denise gets to the main road and meets Captain Spaulding, who offers her a lift. She is so tired that she passes out in the front seat, and Otis appears in the back seat holding a knife. When she wakes up she is strapped to an operating table, with Dr. Satan standing over her beside Earl, who lived through the collapse. The movie ends with Denise screaming and the words "The End?" appearing before the credits.
