What actually happens
A bayonet and a subway ride
In October 1971, Jacob Singer is an infantryman with the 1st Air Cavalry Division, stationed in a Mekong Delta village that gets attacked. Some of his comrades are killed or wounded, and the others start convulsing or freezing in place. Jacob bolts for the jungle, where somebody puts a bayonet in him. He wakes up on a New York subway car, where he sees a tentacle sticking out of a sleeping man. The calendar has jumped to 1975, Jacob is a postal worker who lives in a run-down apartment in Brooklyn with his girlfriend Jezebel, and he keeps seeing his old family, especially his son Gabe, who died in an accident before the war. He also sees faceless vibrating figures and barely survives a car that comes straight at him. His doctor at the VA hospital has no record of him and has supposedly died in a car explosion.
Reading this instead of watching it is the correct decision. Scaredy Cat makes that decision automatic, for free.
A party with a psychic
At a party, a psychic studies Jacob's palm and announces that he is already dead. Jacob takes it as a joke. After he turns down a dance with her, he sees a huge creature dancing with Jezebel before it kills her, and he collapses. At home he gets a dangerous fever, and Jezebel tries to bring it down with a painful ice bath. Jacob then wakes up in a second life with a wife named Sarah and sons who include Gabe. Back in Vietnam, he is evacuated by helicopter while wounded, and the helicopter is shot down.
Everyone in the platoon is having the same nightmare
A former platoon mate, Paul, contacts Jacob to say he is having similar experiences, and then dies when his car explodes. After the funeral the other surviving platoon members admit they are all seeing horrifying things too. They think the army did something to them in secret, so they hire a lawyer named Geary. He drops the case after reading military files that say the soldiers never saw combat and were discharged for psychological reasons. The others back off, and Jacob suspects they were threatened. Suited men abduct Jacob and try to scare him, and he fights his way free but is injured and almost paralyzed.
The hospital nobody wants to visit
After the men in suits, Jacob wakes up in a hospital that looks like a nightmare, and the staff tell him he has been killed and that this is his home now. This is the scene people remember most. Jacob's chiropractor friend Louis Denardo shows up and heals him, which ends it. Louis then quotes the 14th-century mystic Meister Eckhart, who said that when you make peace with death, the devils tearing your life away turn out to be angels freeing you from the earth.
A chemist with a theory
A distressed man who has been following Jacob introduces himself as Michael Newman. He is the one who pulled Jacob away from Paul's burning car. Michael says he was a chemist in the army's chemical warfare division and designed a drug called the Ladder, which makes people far more aggressive. He claims the drug was given secretly to Jacob's unit before the battle, so some of the soldiers turned on each other in a homicidal frenzy. The story brings back a vision of Jacob in Vietnam, wounded, and this time his attacker is a fellow American soldier. Jacob then returns to his family's old home, and his son Gabe walks him up the stairs toward a bright light.
The ending, explained
Gabe leads Jacob up the staircase into the light, and the movie cuts to a triage tent in Vietnam in 1971. The medics declare Jacob dead. One of them remarks that he fought to stay alive and now looks peaceful. The ending is sad, and it is also gentle, because Jacob finishes it by climbing the stairs with his son.
The post-credits ruling
Nobody has confirmed whether there is a scene after the credits in Jacob's Ladder.
β Spoiled safelyDog status: fine, no dog dies.
