What actually happens
Hide-and-seek in an abandoned convent
In 1974, four eleven-year-olds, Nick McBride, Wendy Richards, Jude Cunningham and Kelly Lynch, are playing hide-and-seek in an abandoned convent. A ten-year-old named Robin Hammond tries to join in, and the group scares her. She falls through a window and dies. The children promise each other never to tell anyone, because they're afraid of getting in trouble. As they run out of the convent, the shadow of someone who saw what happened appears behind them.

One shattered mirror and a lot of ruined yearbook photos
Six years later, Robin's older sister Kim and Robin's fraternal twin brother Alex are getting ready for the senior prom, and the date happens to be the anniversary of Robin's death. Wendy, Jude and Kelly get anonymous obscene calls, and Nick lets his phone ring. Kim and Nick are dating now. Jude is going with the goofy joker Seymour "Slick" Crane, Kelly is going with her boyfriend Drew Shinnick, and Wendy, who is Nick's ex, asks the school bully Lou Farmer in order to embarrass Nick and Kim. In the locker room, an unseen person smashes a mirror, and afterwards each of Wendy, Jude and Kelly finds a yearbook photo of herself stabbed with glass. Meanwhile, Kim and Alex's father, the principal Mr. Hammond, hears that Leonard Murch, a sex offender whom people had unofficially blamed for Robin's death, has got out of a psychiatric facility.
A ski mask and a very busy night
At the prom, a figure in a ski mask reaches Kelly first, using a mirror shard to cut her throat in the girls' locker room after she refuses Drew. Jude and Slick head out to Slick's van, parked beyond the school grounds. The masked killer shows up, stabs Jude in the throat using that same shard, and makes Slick drive off a cliff. Meanwhile, Nick's father, Lt. McBride, who is staking out the prom, is told that the man blamed for Robin's death has been captured.
A mistaken identity at the crowning
Armed with an ax now, the killer pursues Wendy down a deserted school wing, corners her in a utility closet and kills her with the ax. The school janitor, Mr. Sykes, sees it and tries to warn the principal, but nobody believes him because he is drunk. That is the build-up to the scene people dread most, and it comes near the end. Kim and Nick are about to get the prom king and queen crowns, but Lou and his friends tie Nick up and Lou takes his crown. The killer takes Lou for Nick and decapitates him, and Lou's severed head rolls across the stage, and the prom-goers run for the exits.
The ending, explained
Kim frees Nick, and the two of them are confronted by the killer. In the fight that follows, Kim swings the ax and catches the killer on the head. They briefly lock eyes, and the killer runs outside holding the wound. Kim follows, begging the police officers who are approaching not to shoot, and the killer collapses on the ground. He turns out to be her brother Alex, who says tearfully that they killed her, and he dies while Kim holds him, saying Robin's name. Kelly, Jude, Slick, Wendy and Lou are all dead by then.
