What actually happens
A routine night shift with a locked door
On the evening of March 11, 2008, TV reporter Angela Vidal and cameraman Scott Percival spend the night shooting a feature about the night shift at a Los Angeles fire station. The firefighters, Jake and George Fletcher, get an emergency call to an apartment building where the landlord, Yuri, his wife Wanda and a crowd of tenants heard screaming from Mrs. Espinosa's apartment. The tenants include Lawrence, Bernard (who shares his place with Sadie), Randy, Kathy and her daughter Briana, and a couple named Nadif and Jwahir. The crew goes in with two police officers, Danny and James. Mrs. Espinosa attacks, biting James and Fletcher, and Danny shoots her. They find another resident, Elise, in the same state and bring her downstairs. Everyone Espinosa bit gets sick and starts to ramble.

Briana and her sick dog
The authorities and the CDC seal the building and let nobody out. Angela interviews Briana, who says her dog stayed at the vet since he was ill, and Lawrence says the symptoms look like rabies. Angela, Scott, Bernard and Sadie then see a rabid dog maul Randy, and Elise attacks them until Scott kills her to protect himself. CDC officers in hazmat suits come in to test Fletcher and James, who wake up and attack an officer and Lawrence. The surviving inspector says Briana's dog is the reason for the quarantine, because it was infected. Briana turns, bites her mother and runs. The group finds her, she bites Danny, and everyone runs downstairs as the infected break loose, and Kathy is killed. Lawrence infects Nadif and Jwahir.
The basement key is on a long walk
The survivors lock themselves in an upstairs room and find that the inspector and Sadie were both bitten. Bernard tries to leave the building and is shot by a sniper outside. Yuri works out that the basement connects to the sewers, but the inspector turns and bites him, and Wanda refuses to abandon her husband and is bitten by Sadie. Jake, Angela and Scott run. They find the basement key and fight past most of the infected on the way, but Yuri, now infected, bites Jake, leaving only Angela and Scott. The infected drive them up to the attic apartment, where they come across lab equipment and newspaper clippings that belonged to a former tenant. He belonged to a doomsday cult that broke into a military biological facility and stole a weapon called the Armageddon Virus, a mutated and highly contagious form of rabies.
Night vision and a very quiet attic
A trapdoor opens in the attic, and an infected boy swats the camera light out. Scott switches to night vision, and then they hear banging inside the apartment. The noise comes from an emaciated person who does not seem to know they are there, searching the rooms blindly. This is the scene people dread most, and it comes at the very end: Angela and Scott try to sneak away, the person notices them and beats Scott to death, and Angela picks up the fallen camera and sees the person eating him. Angela is then attacked, lets go of the camera and gets dragged away into the darkness.
The ending, explained
Angela and Scott are the last two left alive, and the infected push them into the attic apartment. An emaciated person who seems unaware of them searches the rooms blindly, finds them, and beats Scott to death. Angela picks up the camera, sees the person eating Scott, and is attacked, and she lets go of the camera as she is dragged away into the dark.
