What actually happens
A rafting trip with a bad ride home
Sarah is picked up after a whitewater rafting trip with her friends Juno and Beth by her husband Paul and their daughter Jessica. Paul is distracted on the drive, and the car crash that follows kills both him and Jessica. Sarah wakes up in the hospital and collapses when Beth tells her. A year later, Sarah, Juno and Beth meet again to go spelunking in the Appalachian Mountains of North Carolina, together with the sisters Sam and Rebecca and a newcomer named Holly. They hike to the cave entrance and descend. Sarah gets stuck in a narrow tunnel and gets free before it collapses, which blocks the way back. Then Juno admits that she has taken the group into an unexplored cave system rather than the mapped one they reported, so no rescue party would know where to look.

Holly sees sunlight and is wrong
The group looks for another exit and comes across abandoned climbing gear and a cave painting that hints at a way out. Holly thinks she sees sunlight, runs ahead, drops into a hole and breaks her leg. Sarah wanders away while the others tend to Holly, and spots a pale humanoid creature at a pool, drinking, which runs away when it notices her. Later, they find a den full of animal bones. A creature they name a Crawler attacks them and kills Holly. Sarah runs, falls down a hole and passes out. Juno tries to keep Holly's body from being pulled off and kills a Crawler with her pickaxe. Startled, she then stabs Beth in the neck by mistake. Beth grabs Juno's pendant as she falls, and Juno flees in a panic.
Everyone finds out about the pendant
Sarah wakes up in a den of carcasses and watches Crawlers eat Holly's body. Juno finds cave markings from earlier explorers that point to a possible exit, rescues Sam and Rebecca from a Crawler, and Sam figures out that the creatures cannot see and track people by sound. Sarah finds Beth dying. Beth tells her that the stabbing was Juno's doing and that the pendant was a gift from Paul, which confirms Sarah's suspicion that Juno was having an affair with Paul. Beth begs Sarah to put her out of her misery, and Sarah, unwilling, does it with a rock. This is the scene people dread most, and it happens in the middle of the movie. Sarah then meets several Crawlers and fights them off, falling into a pool of blood. Sam is killed on a chasm crossing by a Crawler on the ceiling, and Rebecca is dragged away.
The ending, explained
Juno finds Sarah again and lies about Beth. Near an exit, Sarah shows Juno the pendant and says she knows about Beth and about Paul. She drives a pickaxe into Juno's leg and leaves her as a group of Crawlers closes in, and Juno is heard screaming as Sarah gets away. Sarah falls into a hole, passes out, wakes up and sees sunlight, then climbs a slope of bones to leave the cave. She reaches her car, drives off and stops to cry. Then she looks up and imagines a bloodied Juno beside her, and she screams. In the UK version, Sarah wakes up still in the cave, sees Jessica smiling, and hears the Crawlers coming closer.

Questions people ask at 1am
Is The Descent scary?
Yes, The Descent is a 7 out of 10. It is claustrophobic and graphic, the dread lasts a few days, and the creatures arrive after the group is already lost.
How does The Descent end?
Sarah leaves Juno to the Crawlers and is the only one to climb out of the cave. She then imagines Juno in her car and screams. In the UK version, she is still in the cave and sees her daughter.
Does the dog die in The Descent?
No dog dies in The Descent. A dead animal that looks like a deer appears on screen, and you don't see how it died.
Is there a post-credits scene in The Descent?
Nobody has confirmed whether The Descent has a post-credits scene, so there is no reliable answer.
What are the Crawlers in The Descent?
They are pale, humanoid, blind creatures that live in the cave and hunt by sound. In The Descent, Sam works out that they track the group by noise.
