What actually happens
Five million dollars and an alarm clock
Cecilia Kass is trapped in a relationship with Adrian Griffin, a wealthy optics engineer who runs a company called Cobalt. One night she drugs him with her own diazepam, slips out of his heavily secured house and, with help from her sister Emily, gets away just after a car alarm wakes him. She hides out with her childhood friend James Lanier, a San Francisco detective, and his teenage daughter Sydney. Two weeks later the news breaks that Adrian has taken his own life. His brother and lawyer, Tom Griffin, calls to say that Adrian left Cecilia five million dollars in his will.

Gaslighting, with an invisible assistant
Cecilia starts to sense that someone else is in the house, and James tells her she is traumatized and paranoid. At a job interview the contents of her portfolio have vanished, and she faints. A doctor says there is a lot of diazepam in her system, and at home she finds the bloody diazepam bottle from her escape. She tells James and Tom her theory: Adrian faked his death and turned his optics skills to become invisible, but Tom brushes it off and says he is glad his brother is gone. Emily gets a nasty email that appears to come from Cecilia, and something unseen hits Sydney, who blames Cecilia, so James asks her to leave. Alone in the attic, Cecilia finds Adrian's phone, which immediately texts her the word surprise. She pours paint down the attic hatch onto an invisible figure and escapes after a violent struggle.
Dinner is served with a knife
Cecilia goes to Adrian's house to search his lab and finds a second invisible bodysuit, which confirms what she has been saying. She hides it in the closet of the bedroom they used to share, and the invisible figure attacks her right away, so she flees and contacts Emily. They meet at a restaurant in the middle of the movie. This is the scene to brace for. The invisible figure slits Emily's throat at the table, then puts the knife into Cecilia's hand, which frames her for the murder. Everything after this is about Cecilia trying to prove what happened.
Not a great week to be pregnant and committed
Cecilia awaits trial in a psychiatric hospital, where a voice says surprise to her at night. She learns that she is pregnant. Tom offers to make her charges go away if she returns to Adrian and raises the baby, which hints that he helped fake the suicide, and he admits that Adrian tampered with her birth control. Cecilia refuses and steals Tom's fountain pen. That night she attempts suicide to draw out the invisible figure, and when it tries to stop her she stabs it with the pen, which makes the suit malfunction. The figure incapacitates the guards and leaves, and Cecilia follows it. It threatens to hurt the people she loves, so she races to James's house, where it is attacking James and Sydney. She shoots it and finds Tom inside the suit. Police then find Adrian alive and tied up in his own house, and he says that Tom held him prisoner. Everyone decides Tom killed Emily as well, but Cecilia thinks Adrian set up his brother.
The ending, explained
Cecilia is cleared of the charges and arranges dinner with Adrian at his house, wearing a wire while James listens from a few blocks away. She promises to patch things up with him, but only after he admits to murdering Emily. Adrian keeps blaming Tom and claims the experience changed him, but he says surprise, which tells her he was the invisible man all along. Cecilia goes to the bathroom, puts on the second suit she hid earlier, and slits his throat, and the nearby security camera makes it look like he did it himself. She calls 911, whispers surprise to him, and he collapses dead. When James arrives, she confirms what the camera shows. He notices she is carrying the spare suit, agrees to keep quiet about it, and lets her go.
