What actually happens
Fired by a man named Harvey
Elisabeth Sparkle is a faded Hollywood film star who has hosted a long-running aerobics show on television. On her fiftieth birthday, the producer, Harvey, fires her because of her age. She is so distracted by the sight of workers taking down a billboard of her own face that she crashes her car. At the hospital, a young male nurse slips her a flash drive advertising a black market drug called The Substance, which promises a younger, more beautiful and more perfect version of herself. Elisabeth takes about as long to decide as it takes to get home and place the order.
Elisabeth injects herself with the single-use serum and convulses, and a younger woman named Sue climbs out through a slit in her back. The rules are that the two bodies swap consciousness once a week while the other lies unconscious and is fed intravenously, and that Sue needs daily injections of stabilizer fluid taken from the original body or she deteriorates. Sue auditions to replace Elisabeth, becomes a sensation overnight, and Harvey offers her the New Year's Eve show. Sue enjoys her new life. Elisabeth shuts herself in and hates herself. When Sue takes extra stabilizer fluid to delay a switch for a man she has brought home, Elisabeth's right index finger ages rapidly. The supplier tells her that delaying the swaps will cause irreversible, rapid aging.
A roommate situation that ends badly
Elisabeth and Sue begin to feel like separate people, and they dislike each other more each week. Elisabeth resents Sue for ignoring the schedule, which makes her age faster, and Sue is appalled by Elisabeth's binge eating. After one especially destructive episode, Sue hoards stabilizer fluid and won't switch back. Three months later, the day before the New Year's Eve show, her supply runs out and the supplier tells her she has to switch back. Elisabeth finds herself turned into a hunched, deformed hag. To stop Sue from aging her any further, she orders a serum made to end her. She stops before finishing the injection, still wanting Sue's fame, and revives Sue, so both are awake. Sue works out what Elisabeth tried to do, beats her to death, and leaves to host the show.
The New Year's Eve show goes badly
With Elisabeth gone and most of the serum already used, Sue's body falls apart quickly. She tries to make a new version of herself from the leftover substance, even though it is meant for a single use. The result is a mutated creature with both Elisabeth's and Sue's faces, called Monstro Elisasue. She covers her face with a mask made from an Elisabeth poster and tries to host the show anyway. This is the scene people brace for, and it comes near the end. The audience erupts into chaos. Someone in the crowd decapitates her, and an even more mutated head grows back. Then one of her arms breaks and sprays blood across the audience and the studio.
The ending, explained
Monstro Elisasue flees the studio and collapses outside, where she bursts into a pile of viscera. Elisabeth's original face peels off the gore and crawls up to the star with her name on the Hollywood Walk of Fame, which has been neglected. She smiles as she imagines people admiring her, and then she melts into a puddle of blood. A sidewalk scrubber cleans it up the next day. Elisabeth, Sue and Monstro Elisasue are all dead by the end of the movie.
The post-credits ruling
