What actually happens
One Wish Willow, no refunds
Baron Bailey, who goes by Bear, has a crush on his friend Nikki Freeman. They work at a music store with their friends Ian and Sarah. One night Bear comes home and finds his cat, Sandy, dead after eating oxycodone. He goes to a crystal shop meaning to buy Nikki a new necklace, and buys instead a One Wish Willow, which is a toy that promises a single wish when it is broken. That night Nikki asks whether he likes her, and Bear can't say yes. Frustrated, he breaks the willow and wishes for Nikki to love him above everyone else on earth. She reappears at once and asks to stay with him, claiming that her father, whom she barely speaks to, has cancer and is dying. In bed she kisses him, then screams and pulls away.
Reading this instead of watching it is the correct decision. Scaredy Cat makes that decision automatic, for free.
The sandwich contains a cat
The next day, Nikki builds a memorial from Sandy's remains and blames her odd behavior on MDMA. She confesses that she likes Bear too, and they begin dating. Then Ian calls and tells Bear that Nikki had said she thought of him as her little brother shortly before they got together, and that her story about her father's cancer was a lie. When Bear asks her, she gets defensive and angry. That night he wakes up to find her watching him sleep, crying and accusing him of not loving her. The following day Nikki has taped the door shut, and the sandwich she made for Bear contains Sandy's remains. Bear calls the willow's customer service, and the representative says the wish only ends when Bear dies, then puts a screaming Nikki on the line. Bear hangs up in a panic and goes home, where Nikki has spent the whole day standing in the same spot, waiting, covered in bodily fluids.
A game of Jenga with consequences
Nikki insists on going with Bear to a party at Ian's house, even though Ian doesn't want her there. During a drinking game with Jenga blocks, she reads a violent retelling of Hansel and Gretel that she wrote. Bear draws a block that dares him to kiss the person on his left, who is Sarah. Nikki moves Sarah out of the way and kisses Bear herself, then screams and hurts herself with a broken bottle. Bear drives her to the hospital, but she refuses treatment, so he takes her home.
Sarah picks the wrong place for a chat
That night Sarah texts Bear to meet her at the park. As he tries to leave, Nikki says her obsessive side is asleep and asks Bear to kill her. He says no, and takes it personally that she would pick death over him. At the park, Sarah tells Bear that Ian and Nikki have been sleeping together on and off for two years, and she worries Nikki is using Bear to get back at Ian. Sarah then hints that she likes Bear herself, and Nikki smashes through the car window and slams Sarah's head into a brick until she dies. The scene people dread most is the park, and it comes late in the movie. The two of them move the body, and Nikki sends Bear home and says she will get rid of it herself.
The ending, explained
Bear buys the remaining One Wish Willows to undo his wish, but he can't break one. He tells Ian everything, and Ian sarcastically wishes for a billion dollars, which rains from the ceiling. Bear goes home with the last willow and finds Sarah's mutilated body. Nikki, dressed as Sarah, threatens Bear and herself with a gun. Ian walks in, and Nikki shoots and kills him. Bear locks himself in the bathroom, prepares to shoot himself, and swallows the remaining oxycodone instead. He changes his mind and tries to make himself throw up, but Nikki breaks the last willow. Under the wish, Bear leaves the bathroom and kisses her before he dies. Nikki prepares to shoot herself, but his death releases her from the wish. She sees the damage around her and wails.
